Medicare Supplement policies have exclusions, limitations, reductions of benefits and terms under which the policy may be continued in force or discontinued. Unless specifically stated otherwise, the policy does not cover or pay for any Hospital, Skilled Nursing Facility or other health care institution, health care, treatment, service or supply or any portion of a service or supply that is not a Medicare Eligible Expense. The policy will not duplicate any benefit paid by Medicare or cover any charge that a person is not legally obligated to pay or that would not be made in the absence of insurance. UNDER AGE 65: In some states, Medicare Supplement Insurance plans may be available to qualified individuals under age 65 who are eligible for Medicare by reason of disability and/or end stage renal disease.
AN OUTLINE OF COVERAGE IS AVAILABLE UPON REQUEST AND SHALL BE PROVIDED AT TIME OF APPLICATION.
To be eligible for Medicare Supplement, you must have Medicare Part A and Medicare Part B to enroll in this plan. You must continue to pay your Medicare Part B premium each month, unless it is otherwise paid for under Medicaid or by another party. A Medicare Supplement policy/plan only covers one person. Plans may vary by state.

Some plans may not be available. Only applicants first eligible for Medicare before 2020 may purchase Plans C and F.
In South Dakota, premiums or rates may increase for other reasons such as an increase applicable to all persons covered under this type of policy or certificate in this state.
In Idaho, the advertised policies include Limitations and Exclusions. Policies do not cover or pay for any of the following:

Any hospital, skilled nursing facility or other health care institution confinement that is not covered under Medicare or for which Medicare makes no payment, except as may otherwise be provided in the policy.
•
Any health care treatment, service or supply that is not covered under Medicare or for which Medicare makes no payment, except as may otherwise be provided in the policy.
•
Any benefits which duplicate benefits provided by Medicare.
•
Any charge that a person is not legally obligated to pay or that would not be made in the absence of insurance.
In Texas, no individual can be denied any Medicare Supplement policy sold by any Medicare Supplement issuer if the application is submitted during the six-month period beginning on the first day of the month in which Medicare Part B becomes effective. If you are on Medicare under age 65, you will also have a six-month open enrollment period beginning the month you turn age 65.
In Tennessee, premiums or rates increase annually with increasing age and may increase for other reasons such as an increase applicable to all persons covered under this type of policy or certificate in this state.
Maryland law requires that certain Medicare Supplement plans be made available to individuals who are under age 65 and are eligible for Medicare due to disability.
In Florida: An individual cannot be denied any Medicare Supplement policy sold by an issuer if the application is submitted during the “Open Enrollment Period” (the six-month period beginning with the first month in which an individual who is 65 years or older first enrolls for benefits under Medicare Part B). (1) Our Medicare Supplement plans are also offered to individuals under age 65 who are entitled to Medicare benefits because of disability or end-stage renal disease. These individuals are also entitled to open enrollment and cannot be denied any Medicare Supplement policy sold by an issuer if the application is submitted during the “Open Enrollment Period.” Additionally, they have a second “Open Enrollment Period” when they turn age 65; (2) An individual who is 65 years of age or older, or under 65 years of age and eligible for Medicare by reason of a disability or end-stage renal disease, who is enrolled in Medicare Part B, cannot be denied any Medicare Supplement policy if they apply during the two-month period following termination of coverage under a group health insurance policy. The advertised policies have exclusions, reductions and limitations. Unless specifically stated otherwise, this policy does not cover or consider for payment any service or supply or any portion of a service or supply that is not a Medicare Eligible Expense, nor will this policy duplicate any benefits paid by Medicare. Provided premiums are paid and there is no misrepresentation on the application, the advertised policies are guaranteed renewable for life.
